Navigating the ACCME Standards Update: A CPD Professional’s Guide to Compliance
Expert Perspectives · Madelyne Fabrizio, BA, CHCP · 2024-10-29
As a seasoned CPD professional at i3 Health, I’ve witnessed numerous changes in the medical education landscape, but the 2020 ACCME standards update was a game-changer. The shift from “Standards for Commercial Support” to “Standards for Integrity and Independence” isn’t just a name change – it’s a fundamental reimagining of our approach to CME. Key changes include an expanded definition of “ineligible companies,” stricter relevant financial relationships policies, enhanced transparency requirements, and a heightened focus on content validity. These updates have pushed us to be more vigilant in our processes, from vetting speakers to reviewing content.
While implementing these changes hasn’t been easy – many of us have had to overhaul our systems and retrain our teams – I see them as an opportunity to elevate the quality and integrity of our programs. They encourage us to forge deeper partnerships with our faculty and to be more creative in delivering unbiased, high-quality education. As we continue to adapt, it’s crucial to stay vigilant against slipping back into old habits. I’ve included some ways I’ve navigated this within my own organization, so you know you’re not alone!
The Evolution of Standards: More Than Just a Name Change
When I first heard about the transition from “Standards for Commercial Support” to “Standards for Integrity and Independence,” I knew we were in for more than just a cosmetic update. This change signaled a broader, more holistic approach to maintaining the quality and objectivity of accredited continuing education.
When I first started in this field, our focus was primarily on managing commercial support and avoiding conflicts of interest. While these remain crucial, the new standards have pushed us to consider a wider range of factors that could potentially compromise the integrity of our educational programs. It’s been a welcome shift, in my opinion, as it aligns more closely with the complex realities of modern healthcare education.
ACCME Standards Overview
Streamlining: A Double-Edged Sword
The reduction from six standards to five initially seemed like a simplification. However, as I delved deeper, I realized that each standard now encompasses more ground. It’s reminiscent of when I downsized my office after going remote – fewer filing cabinets, but each one packed with a bit more information.
This consolidation has forced me to take a more holistic view of our CME programs. Instead of treating each standard as a separate checkbox, I now find myself considering how each aspect of our educational activities intersects with multiple standards simultaneously. It’s been challenging but ultimately has led to more cohesive and thoughtfully designed programs.
The Great Ineligibility Debate
One of the most significant changes I’ve grappled with is the clear definition of “ineligible companies.” I remember the days when the lines between eligible and ineligible organizations were blurrier, often leading to lengthy discussions and occasional disagreements within our team.
The new, more precise definitions have been a benefit in many ways. They’ve eliminated much of the guesswork and debate. However, they’ve also meant severing some long-standing partnerships that no longer meet the criteria. It’s been a bittersweet process, but ultimately, I believe it’s strengthened the integrity of our programs.
Disclosure: Devil in the Details
If there’s one area where I’ve seen the most day-to-day impact, it’s in the enhanced disclosure requirements. The new standards provide much more specific guidance, which has been both a blessing and a challenge.
On one hand, the clarity has reduced ambiguity and helped ensure consistency across our programs. On the other, it’s required us to revamp of our disclosure forms and processes. It took a while to revise forms, update our database, and retrain team members, all while remembering not to fall into the same habits as before. Though it was a lot to process, the result is a more transparent and trustworthy disclosure process.
Mitigating Financial Relationships: From Art to Science
In the past, identifying and resolving conflicts of interest often felt more like an art than a science. The new standards have pushed us towards a more systematic approach to mitigating relevant financial relationships.
I’ve found this shift particularly invigorating. It’s challenged me to develop more comprehensive strategies for identifying potential issues and addressing them proactively. We’ve implemented a more thorough multi-step review process that has significantly improved our ability to maintain the integrity of our educational content.
Commercial Support: New Rules, Same Principles
While the core principles around commercial support haven’t changed dramatically, the updates and clarifications in the new standards have prompted a thorough review of our practices.
I’ve used this as an opportunity to revisit all our commercial support agreements. It’s allowed us to ensure we’re not just compliant, but truly embodying the spirit of the standards in all our interactions with supporters.
The Ancillary Activities Conundrum
The introduction of a new standard addressing ancillary activities was a welcome surprise to me. We’ve always offered additional resources alongside our CME programs, so making the requirements of hosting them together official was great to see, in my opinion, so there are no gray areas anymore.
Reviewing this standard to ensure we are compliant has also pushed us to be more intentional about how we present these activities and has led to improved clarity for our learners.
Embracing Change: A Continuous Journey within the ACCME Standards Update
As I reflect on the past few years navigating these new standards, I’m struck by how much they’ve pushed me to grow as a CPD professional. They’ve challenged me to question long-held assumptions, develop new skills, and approach our educational programs with fresh eyes.
Implementing these changes hasn’t always been easy and continues to be something I review to confirm I’m on the right path. There have been late nights poring over the standards, spirited debates with colleagues, and moments of frustration as we’ve worked to align our practices with the new requirements.
But through it all, I’ve been reminded of why I entered this field in the first place – to ensure that healthcare professionals have access to high-quality, unbiased education that ultimately improves patient care. These new standards, while challenging, have undoubtedly moved us closer to that goal.
As we continue to refine our processes and adapt to these changes, I’m excited about the future of CME. These standards have set a new bar for integrity and independence in our field, and I believe we’re all better for it.
To my fellow CPD professionals: embrace these changes, lean into the challenges, and remember that our work has a real impact on healthcare professionals and the patients they serve.
